1. Agreement to terms
These Terms and Conditions ("Terms") constitute a binding agreement between you and Inform ("Inform", "we", "us", or "our") governing your access to and use of the Inform mobile application and related services (collectively, the "Services").
Inform provides a digital platform for physiotherapy and rehabilitation, enabling patients to manage health profiles, treatment cases, rehabilitation programmes, and communication with authorised care team members.
By creating an account or using the Services, you confirm that you have read, understood, and agree to be bound by these Terms. If you do not agree, you must discontinue use of the Services.

2. Eligibility and account responsibility
You must be at least 16 years of age to use the Services, or have obtained parental or guardian consent where required by applicable law.
You agree to provide accurate registration information and to maintain the currency of your account details. You are responsible for safeguarding your login credentials and for all activity conducted under your account.
3. Clinical and health disclaimer
The Services support rehabilitation management and care coordination. They do not constitute medical advice, diagnosis, or treatment, and are not a substitute for consultation with a qualified healthcare professional.
Clinical decisions and treatment recommendations are made by authorised practitioners engaged in your care. Inform operates as a technology platform and does not itself provide medical or allied health services.
Automated features, including AI-assisted intake and chatbot functionality, are provided for informational and administrative support only. Such features may not be complete, current, or appropriate for your individual circumstances.
In the event of a medical emergency, contact your local emergency services immediately.
4. Account access
Account registration may be completed using email and password, Google Sign-In, or Sign in with Apple, where available. You must not permit unauthorised access to your account or use another person's account without permission.
We may suspend or terminate your account where we reasonably determine that you have breached these Terms, misused the Services, or created risk to Inform, other users, or third parties.
5. Permitted use
Subject to these Terms, Inform grants you a limited, non-exclusive, non-transferable, revocable licence to access and use the Services for personal, non-commercial rehabilitation and care-related purposes.
You must not:
- use the Services for unlawful, harmful, or fraudulent purposes;
- attempt to gain unauthorised access to systems, accounts, or data;
- interfere with or disrupt the integrity or performance of the Services;
- upload content that is unlawful, defamatory, infringing, or otherwise objectionable;
- reverse engineer, scrape, or exploit the Services except as permitted by law; or
- use the Services on behalf of another person without proper authority.
6. Subscriptions and payments
Certain features may require a paid subscription. Pricing, billing intervals, and plan inclusions are displayed at the point of purchase within the application or applicable checkout flow.
Payments are processed by third-party payment providers. By subscribing, you authorise the relevant provider to charge your nominated payment method in accordance with the selected plan.
Subscriptions may renew automatically unless cancelled in accordance with the terms of the applicable app store or payment provider. Refunds, where available, are subject to applicable law and the policies of the relevant platform.
7. User content
You may submit messages, health information, media, documents, and other materials through the Services ("User Content"). You retain ownership of your User Content.
You grant Inform a licence to host, store, process, display, and disclose User Content as reasonably necessary to operate the Services, facilitate your care, and comply with legal obligations.
You represent and warrant that you have the right to submit User Content and that its submission does not infringe the rights of any third party or violate applicable law.
8. Care team services
Your care team may comprise physiotherapists, clinicians, and other authorised professionals using the Inform platform. Professional advice and clinical judgement rest with those practitioners, not with Inform as platform operator.
9. Intellectual property
The Services, including all software, design elements, branding, content libraries, and underlying technology, are owned by Inform or its licensors and are protected by applicable intellectual property laws. No rights are transferred to you except as expressly granted in these Terms.
10. Privacy
Our collection and handling of personal information is described in our Privacy Policy, which forms part of your agreement with Inform. By using the Services, you acknowledge that you have reviewed the Privacy Policy.
11. Termination and account deletion
You may delete your account through Profile → Delete Account within the application, following the confirmation steps required. Account deletion is permanent.
If you cannot access the application, you may submit a deletion request to hello@physioteneka.com from your registered email address with the subject line "Account Deletion Request". Limited information may be retained as described in our Privacy Policy.
We may suspend or terminate access to the Services at any time where permitted by law, including for breach of these Terms or to protect the platform and its users.
12. Disclaimers
To the fullest extent permitted by law, the Services are provided on an "as is" and "as available" basis without warranties of any kind, whether express, implied, or statutory, including warranties of merchantability, fitness for a particular purpose, or non-infringement.
We do not warrant that the Services will be uninterrupted, error-free, or free from harmful components.
13. Limitation of liability
To the fullest extent permitted by law, Inform and its affiliates, officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of profits, data, goodwill, or other intangible losses arising from or related to your use of the Services.
Nothing in these Terms excludes, restricts, or modifies any right or remedy that cannot be excluded under applicable law, including non-excludable consumer guarantees under Australian law.
14. Third-party services
The Services may integrate with third-party platforms, including Google, Apple, Firebase, and payment processors. Your use of such services is subject to their respective terms and privacy policies. Inform is not responsible for third-party services.
15. Amendments
We may amend these Terms from time to time. Material changes will be notified through the application, by email, or by publication of updated Terms with a revised effective date. Continued use of the Services following the effective date constitutes acceptance of the amended Terms.
16. Governing law and disputes
These Terms are governed by the laws of Australia, without regard to conflict of law principles, except where mandatory local consumer protections apply.
Before initiating formal proceedings, you agree to contact us to attempt to resolve any dispute informally.
